What is a Trip?

A Trip is your transportation activity from the time you're picked up for the first time of the day until you are dropped off at at your final destination.

Your Trip consists of an A leg, B leg and possibly C and D leg if you have more than one appointment and/or destination scheduled.

What is a Trip?

    A "Trip Leg" refers to a segment or part of the day's trip. The first trip leg will involve traveling from your home or healthcare facility to your medical appointment. This is called the A Leg. 

If you will need transportation from the facility you were initially transported to, your trip will consist of a B Leg.

If your B Leg is not your final destination, your trip will consist of a C Leg.

There are instances that a member has a D Leg and further.

 Each trip leg is assigned a cost that corresponds to the distance covered. This is calculated using a set rate per mile + base rate pricing.  

Dead head is calculated using zone based pricing. Please see price list below.

Transportation/Delivery Programs

Ride Share Program

Non-Emergency Medical Transportation. Louisiana Sales Tax will be added at checkout.

Ambulatory Trip (Base Rate)

$23.00/leg

  • Members that need transportation to an appointment without assistance first 5 miles included

Ambulatory Trip (Milage Rate)

$1.65/mile

  • Members that need transportation to an appointment without assistance  (first 5 miles included with Base Rate each additional mile is $1.65 per mile)

Dead Head Ambulatory (Milage Rate)

$.75/mile

  • Any Ambulatory trip that starts 20  or more miles from our base at 2217 Lee Street Alexandria, LA 71301 will be charged an additional rate of $.75 per mile for distance driven to pick up a member and also for the return mileage back to our base

Wheelchair Trip (Base Rate)

$28.00/leg

  • Wheelchair Members that need transportation to an appointment with assistants (first 5 miles included with Base Rate each additional mile is $1.85 per mile)

Wheelchair Trip (Mileage Rate)

$1.85/mile

  • Wheelchair Members that need transportation to an appointment with assistants (first 5 miles included with Base Rate each additional mile is $1.85 per mile)

Dead Head Wheelchair (Mileage Rate)

$1.00/mile

  • Any Wheelchair trip that starts 20 miles from our base at 2217 Lee Street Alexandria, LA 71301 will charged an additional rate of $1.00 per mile for distance driven to pick up a member and also for the return mileage back to our base

Cancellation and Refund Policy

Cancellation Policy:


Thank you for choosing our Non-Emergency Medical Transportation (NEMT) services. We understand that circumstances may arise where you need to cancel your booked trip. Please review our cancellation policy below:


1. Cancellation 48 hours or more before the scheduled trip:

   - If you cancel your trip at least 48 hours prior to the scheduled trip, you will be eligible for a full refund of the trip cost.


2. Cancellation with less than 48 hours before the scheduled trip:

   - If you cancel your trip with less than 48 hours before the scheduled trip, a cancellation fee of 50% of the total trip cost will be charged.


Refund Process:

- If you are eligible for a refund according to the above cancellation policy, we will process the refund within [3] business days after receiving the cancellation request.

- Refunds will be issued to the original payment method used during the booking process.


Cancellation Request:

- To cancel your booked trip, please contact our customer support team at [318.625.7021] or email us at [Booking@unitedcaretransportation.com]. Provide your booking details, including the trip date and time, as well as your contact information.


Note:

- Our cancellation policy applies to regular trips. Special circumstances or events may have different cancellation policies, which will be communicated at the time of booking.

- We recommend canceling your trip as early as possible to avoid any cancellation fees.


 

  1. No-Show Fees: If you're not at the designated pickup location within a specific time frame, the driver may mark you as a "no-show," and United Care Transportation LLC may charge you a fee for this. The no-show fee is equivalent to the cost of one of your scheduled trip legs.
  2. Refunds: If you cancel a trip within the cancellation window or, if, by the sole fault of our company, you were not able to receive your medical treatment (i.e. the driver was extremely late and caused you to miss your appointment) you may be eligible for a refund. 


Please make sure to review and understand our cancellation policy before booking your trip. If you have any further questions or require assistance, feel free to reach out to our customer support team.
