What is a Provider?

Your provider is the transportation company that will physically transport you to and from your appointment(s). There are several different Non-Emergency Medical Transportation (NEMT) Providers that transport members, your ride will be listed as available or offered to any of these Providers to accept your trip(s). 

What is a Preferred Provider?

In many cases, Medicaid members have the right of choice as to who their transportation provider for NEMT services will be. Medicaid programs generally offer beneficiaries a level of choice and control when it comes to selecting their NEMT Provider. This allows members to have a say in the transportation services they receive and ensures that their specific needs and preferences are considered.  

Preferred Provider:The company that YOU choose to accommodate

 your trip(s).

 If you have enjoyed your ride(s) with us and would like to choose United Care Transportation as your Preferred Provider, meaning that your trip will be offered to and/or automatically scheduled with us, here's how you do it.

1.  When you schedule your transportation request with your health plan, please request that United Care Transportation be listed as your Preferred Provider. 
2.  Member Services will give you a confirmation number for your upcoming trip.  
3. By filling out the form below, we are alerted that you have made a transportation request and may help in ensuring that your trip is scheduled with us.   If you don't have an email address no worries ,Just call us at (318)625-7021 ext.(2) Our team will be happy to assist you!!
